Ottawa Curling Club : Membership Application

The following OCC membership form can be completed and submitted online. The information is emailed to the OCC Manager, who will review it and email confirmation to the sender, including the amount due and payment arrangements. Fees are paid subsequently by cheque(s), cash, debit card, or credit card (over the phone or in person).

There is a section for credit card authorization in the application summary, but this has to be completed in writing because the form will not electronically submit or even collect this information (the email is not encrypted). To use this section, it is suggested that you print the application summary and then manually fill in the authorization section before mailing, faxing, or delivering the form to the Ottawa Curling Club.

We also recommend if you are intending to play in one of our prime leagues, that you register your team prior to June 15, 2004. The team registration form is currently available for downloading in PDF or Word format.

Full-time students with ID card pay 200% off of fees (except for the Junior and Little Rock categories), plus the full Capital Levy.

PAYMENT OF FEES

Select the plan and method of payment. Fees can be paid in full by cash, cheque (make cheques out to "Ottawa Curling Club"), debit card or credit card. Installment plans (available for Categories A and B only) are payable by post-dated cheques or by pre-authorized credit card payment.

*Payment Plan:
Payment in Full:	One payment of the Total Due.
Early Payment Plan:	Four payments: June 1, July 1, Aug. 1, and Sept. 1, 2004. Divide the Total Due by 4 for each payment.
Late Payment Plan:	Four payments: Sept. 1, Oct. 1, Nov. 1, and Dec. 1, 2004. Add the $10.00 carrying fee to the Total Due, then divide by 4 for each payment.

All fees are due September 1, 2004 (including post-dated cheques under the Late payment plan). An administration fee of $50.00 will be assessed for any membership fees in arrears as of November 1, 2004, and playing privileges suspended November 1. $15.00 charge for NSF cheques.
