Ottawa Curling Club : Membership Application

The following OCC membership form can be completed and submitted online. The information is emailed to the OCC Manager, who will review it and email confirmation to the sender, including the amount due and payment arrangements. Fees are paid subsequently by cheque(s), cash, debit card, or credit card (over the phone or in person).

There is a section for credit card authorization in the application summary, but this has to be completed in writing because the form will not electronically submit or even collect this information (the email is not encrypted). To use this section, it is suggested that you print the application summary and then manually fill in the authorization section before mailing, faxing, or delivering the form to the Ottawa Curling Club.

PAYMENT OF FEES

Select the plan and method of payment. Fees can be paid in full by cash, cheque (make cheques out to "Ottawa Curling Club"), debit card or credit card. Installment plans (available for Categories A and B only) are payable by post-dated cheques or by pre-authorized credit card payment.

*Payment Plan:
Payment in Full:	One payment of the Total Due.
Early Payment Plan:	Four payments: June 15, July 15, Aug. 15, and Sept. 15, 2007. Divide the Total Due by 4 for each payment.
Late Payment Plan:	Four payments: Sept. 1, Oct. 1, Nov. 1, and Dec. 1, 2007. Add the $10.00 carrying fee to the Total Due, then divide by 4 for each payment.

• Class B Preference share:
I hereby acknowledge that my membership in the Ottawa Curling Club carries with it one (1) Class B Preference share in the Ottawa Curling Club Limited. If, for any reason, my membership in the Ottawa Curling Club is terminated, I hereby waive any claim to the said Class B Preference share.
